These forms are provided online as a convenience to participants who need extra copies. Print copies were mailed out to all advisors. All forms are due postmarked by February 15. Should the 15th be a holiday, they must be postmarked the first USPS day after the 15th.
YOUR DELEGATION ADVISOR MUST SIGN ALL REGISTRATION AND APPLICATION FORMS. Your parents must sign leadership applications.
Unsigned forms will be returned for signature.
